Your teeth are composed of a crown and root. The root holds the crown by being embedded in the bone. Like tooth roots, dental implants are titanium or ceramic support structures that are embedded into the bone of the jaw to support a dental restoration.
We utilize dental implants in a variety of ways to replace single teeth, several teeth, and even complete upper and lower arches of teeth. In some instances a patient may be missing several teeth, but has healthy teeth surrounding the space. Implants are extremely versatile, in that only two to three implants are needed to restore long spans of missing teeth through the concept of bridging. Dental implant bridging implies that some tooth crowns will not have “roots” underneath them, but instead, be suspended between implant supported crowns.
An implant that restores a single tooth is an excellent replacement choice, as the restoration will look, feel, and function just like a natural tooth. Additionally, we can often deliver an implant at the same time a tooth is removed — this is known as immediate placement and can be applicable to any tooth in the mouth. However, sometimes missing tooth sites don't have enough bony support for an implant, which may require the addition of grafting before or during implant placement.
In growing individuals full-sized implants are typically not recommended as their position will shift relative to the other teeth. However, at Dream Dental Sedation we offer mini-implant solutions to temporarily restore teeth until a child is fully grown. This is an excellent treatment option for congenitally missing premolars and lateral incisors during the teenage years.
We have the capability to place immediate implants for all types of teeth the same day they are removed. Often more convenient, same day placement of the dental implant has some distinct advantages. When the tooth is removed and the site is allowed to heal without support, a defect can develop. Also, the additional healing time can delay the final result. Therefore, immediately after tooth removal, implant placement may provide a more aesthetic outcome faster.
A couple types of full-mouth implant approaches are known to improve quality of life as well as jawbone health, compared to the constant hassle and discomfort of conventional dentures or a mouth of broken-down teeth. This implant bridge is a light-weight full mouth implant solution that stays in place like normal teeth. Depending on your unique situation, 4 to 6 implants can be placed to support each dental arch.
Implant retained dentures are another way to help with dental implant stability. The implants hold the dentures in place, while allowing them to be removed and cleaned after meals.
Dream Dental Sedation surgically places and restores our own implants. Sometimes the risks of placing the implant near vital structures within the bone are very high, or the implant site may require extensive grafting prior to placement.
Implants can be placed at the time of extraction (immediate placement), barring an infection or significant loss of bone. Or they can be placed four to six months after socket preservation is done, which is the traditional route. We can provide both implant procedures; however, we always try to do immediate placement as it saves four to six months of healing and can better preserve the soft tissue profile that used to exist around the natural tooth.
Although there are other tooth replacement options available like dental bridges and dentures, the reasons why dental implants are the regarded as the best standard of care are numerous.
When taken care of, implants are the best long-term solution for replacement of missing teeth and provide similar esthetics and function as the natural teeth they replace. While there are other procedures that can be done to replace a missing tooth, these options usually involve reducing the size of the teeth next to missing tooth space and placing a bridge between the prepped teeth, or taking something in and out of your mouth such as a partial denture.
After the implant procedure and a healing period of four to eight months, the bone will “ankylose” into the threads of the implant, and the implant will become rigidly anchored into the bone (also known as osseointegration). After a set healing period, the implant is uncovered and an implant-retained crown can be screwed internally into the implant. The fully restored implant looks and acts like a tooth — almost like it was never gone.
We require a thorough dental exam and look at your health history prior to any dental implant placement. Certain medications or medical conditions, such as diabetes, or habits like smoking or vaping, reduce the success rate of implant integration and must be discussed when deciding to go ahead with placement. At the appointment, we’ll also discuss the process and what to expect post-op. X-rays of the implant site will be taken as well as diagnostic 3D digital image so that we can determine what size of implant to use. Then we will have you come in and see the digital plan.
Dream Dental Sedation offers IV sedation for all dental surgery procedures. While not required for implant placement (most patients choose to simply be numbed with local anesthetic), it is an option that can be discussed at the consultation appointment. Most of the time surgical implant placement of one or two implants takes no longer than 60 minutes.
Once you are numbed, we make a small incision to displace the soft tissue at the site where the implant will be placed so that we can create a hole into the bone where the implant will be inserted. This hole is slightly smaller in diameter than the implant itself to allow the implant to be torqued into place. Depending on the surgical site bone thickness, sometimes a graft must be placed to thicken the bone around the implant as it heals. If this is not required, then a healing abutment is placed over the implant, creating a tissue profile as the implant heals.
At six months post-operation, the healing abutment will be removed and a digital impression scan body is placed within the implant so that a digital image can be made for fabrication of a crown for the implant. This can be done chairside in one appointment.
